Abstract: Abstract: Objective To investigate the efficacy of the combination of glutamine dipeptide and octreotide in treating severe acute pancreatitis (SAP) - related lung injury patients and their effects on serum Toll like receptor 4 (TLR4) and matrix metalloproteinase-9 (MMP-9) levels. Methods: Ninety-four patients with SAP-associated lung injury admitted to our hospital from June 2022 to June 2024 were selected and randomly divided into an observation group and a control group, with octreotide treatment in the control group and prolylglutamine treatment in the observation group (on the basis of the control group). The efficacy, the changes in severity of the condition (Balthazar CT score, APACHE II score, MLIS score), serum TLR4, MMP-9, and blood gas indicators arterial partial pressure of oxygen (PaO2), blood oxygen saturation (SaO2), and oxygenation index before and after treatment were compared between the two groups. Results The observation group showed better efficacy (P<0.05). Compared with before treatment, the Balthazar CT score, APACHE II score, MLIS score, serum TLR4, MMP-9 levels in both groups obviously decreased after treatment, while PaO2, SaO2, and oxygenation index obviously increased, and the observation group was better (P<0.05). Conclusion: The combination of glutamine dipeptide and octreotide is effective in treating patients with SAP related lung injury, it can improve the clinical symptoms of patients, reduce lung injury, inhibit inflammatory reactions, improve blood gas indicators, and promote patient recovery
