Abstract: Objective:To analyse the application value and related factors for left ventricular? global longitudinal strain in early cardiac function injury in patients with diabetes mellitus by two-dimensional speckle tracking echocardiography. Methods:Forty-five patients with diabetes who came to our endocrinology department from January 2023 to January 2024 were selected as the study objects.The clinical and echocardiographic data of all patients were complete.According to the level of HbA1c, patients were divided into group A (≤11.5%) and groups B (> 11.5%) .The clinical data and echocardiographic data from the two groups were compared.The relationship between clinical, echocardiographic data and left ventricular global longitudinal strain was analyzed by univariate and multivariate linear regression.ROC curve was plotted to evaluate the diagnostic efficacy of HBA1c in LVGLS.Results :Of the 45 subjects with normal LVEF levels, LVGLS decreased in 24 (53.3%).Compared with group A, body weight, LVGLS were? reduced and HbA1c,IVST were significantly increased in group B , and the differences were statistically significant (all P < 0.05).There were no significant differences in other clinical and echocardiographic data.By linear regression analysis, it was found that HbA1c had a linear relationship with LVGLS (β value was -0.511).Through ROC curve analysis, it was found the area? under the curve (AUC) for diagnosing LVGLS was largest when the cutoff value of HbA1c was 12.65%, with an area of 0.746, sensitivity of 100% and specificity of 75%.Conclusions:For diabetic patients, body weight, IVST,LVGLS may be related to HbA1c; Increased HbA1c in diabetic patients is an independent risk factor for LVGLS. HbA1c is of high diagnostic value in determining whether LVGLS decreases.
