Abstract: Gastric cancer ranks as the fourth leading cause of cancer-related mortality worldwide. High mobility group box protein 1 (HMGB1) is significantly overexpressed in gastric cancer cells and tissues, serving as a potential biomarker for the development of this malignancy. HMGB1 facilitates gastric cancer progression by promoting proliferation, migration, invasion, and epithelial-mesenchymal transition through diverse signaling pathways while concurrently suppressing apoptosis. Within the tumor microenvironment, HMGB1 contributes to tumor progression by modulating the microenvironment via exosomal pathways. Additionally, non-coding RNAs play a regulatory role in modulating HMGB1 activity in gastric cancer. Numerous natural inhibitors have been identified to target HMGB1, demonstrating its promising potential as a therapeutic target in gastric cancer management. This review summarizes the molecular mechanisms underlying HMGB1's role in gastric cancer and explores recent advancements in HMGB1-targeted therapeutic strategies.
