Abstract: Polycystic ovary syndrome (PCOS) is a common reproductive and endocrine-metabolic disorder in women of childbearing age, characterized by insulin resistance and often accompanied by obesity and chronic low-grade inflammation. The occurrence and development of PCOS are closely related to dysfunction of adipose tissue, but the pathogenic mechanism is not yet fully elucidated. Isthmin1 (ISM1) is a novel insulin-like adipocyte factor that can improve insulin resistance and enhance insulin sensitivity by regulating glucose uptake in adipose tissue and muscle through non-insulin-dependent pathways. ISM1 also participates in regulating PCOS by inhibiting insulin-induced lipid synthesis. ISM1 can predict abdominal fat distribution and is a potential biomarker for PCOS-related obesity. Furthermore, ISM1 inhibits the activation of the NF-κB inflammatory pathway and the expression of pro-inflammatory factors to participate in the inflammatory state of PCOS. This article mainly elaborates on the role mechanism of ISM1 in the development of PCOS from the perspectives of insulin resistance, obesity, and inflammation, with the aim of providing new ideas for the diagnosis and treatment of PCOS.
