摘要:目的 比较黏液型与非黏液型肺炎链球菌的临床感染分布特征及体外药敏实验中对常用药物的耐药性。方法 收集2023年1月至2024年6月厦门市某医院临床标本分离的疑似肺炎链球菌,采用VITEK2-Compact全自动鉴定药敏分析仪及相应配套GP鉴定卡和质谱仪鉴定出291株肺炎链球菌,GP68药敏卡片和E-test药敏纸条、KB药敏纸片等进行抗菌药物敏感性实验。结果 非黏液型肺炎链球菌的感染人群以儿童占比最高,成人及大龄儿童较易感染黏液型肺炎链球菌,两种表型的肺炎链球菌均以肺部感染为主。非黏液型肺炎链球菌对万古霉素、利奈唑胺、青霉素、阿莫西林、头孢噻肟、头孢曲松、厄他培南、氧氟沙星和氯霉素、左氧氟沙星、莫西沙星和氧氟沙星的敏感率较高;对复方新诺明、克林霉素、红霉素、四环素的耐药率高,对美罗培南敏感率仅为63.1%。黏液型肺炎链球菌对青霉素、阿莫西林、头孢噻肟、头孢曲松、厄他培南、万古霉素、利奈唑胺、左氧氟沙星和莫西沙星完全敏感,对复方新诺明、克林霉素、红霉素、四环素的的耐药率也低于非黏液型肺炎链球菌。结论 实验室应提高黏液型肺炎链球菌的检出能力,黏液型肺炎链球菌较非黏液型肺炎链球菌对常见抗菌药物的敏感性更高,临床医生需根据药敏结果选用抗菌药物,延缓耐药的发生。
Abstract: Objective To compare the clinical distribution characteristics and the drug resistance to common drugs in vitro of mucoid Streptococcus pneumoniae with that of non-mucoid. Methods Suspected Streptococcus pneumoniae was isolated from clinical samples in a Xiamen hospital from January 2023 to June 2024. 291 strains of Streptococcus pneumoniae were identified by VITEK2-Compact and GP card or mass spectrometer. GP68 card, E-test strip, KB paper and other antimicrobial susceptibility tests were carried out. Results The proportion of non-mucoid Streptococcus pneumoniae infection was the highest in children, adults and older children were more susceptible to mucoid Streptococcus pneumoniae infection, both the two types of streptococcus pneumoniae mainly induce pulmonary infection. Non-mucoid Streptococcus pneumoniae was highly sensitive to vancomycin, linezolid, penicillin, amoxicillin, cefotaxime, ceftriaxone, ertapenem, ofloxacin and chloramphenicol, levofloxacin, moxifloxacin and ofloxacin. The resistance rate to cotrimoxazole, clindamycin, erythromycin and tetracycline was high, and the sensitivity rate to meropenem was only 63.1%. Streptococcus mucoid was completely sensitive to penicillin, amoxicillin, cefotaxime, ceftriaxone, ertapenem, vancomycin, linezolid, levofloxacin and moxifloxacin, and the resistance rate to cotrimoxazole, clindamycin, erythromycin and tetracycline was lower than that of non-mucoid. Conclusion The detection ability of Streptococcus mucoid pneumoniae should be improved in the laboratory. Mucoid Streptococcus pneumoniae is more sensitive to common antibiotics than non-mucoid. Antibiotics should be selected according to the results of drug sensitivity to delay the occurrence of drug resistance.
