摘要:目的:总结广西中部少数民族地区坏死性筋膜炎的病原菌分布及药敏特征,为临床早期抗感染治疗提供证据支持。方法:回顾性分析2017年1月至2023年12月柳州市工人医院创伤修复烧伤整形外科收治的69例坏死性筋膜炎患者微生物培养和药敏试验结果。结果:69例病例共培养出77株细菌,其中革兰氏阳性菌24株,阴性菌48株,真菌5株。单微生物感染为主,占66.07%。阳性菌株以金黄色葡萄球菌10例(12.99%)、化脓性链球菌5例(6.49%)多见,阴性菌株以肺炎克雷伯菌13例(16.88%)、大肠埃希菌10例(12.99%)为主。多重耐药菌18株,占总菌株的10.39%。革兰氏阳性菌对万古霉素、利奈唑胺、替加环素等更敏感,而革兰氏阴性菌对美罗培南、头孢西汀、厄他培南更敏感。结论:在广西中部少数民族地区,肺炎克雷伯菌是坏死性筋膜炎最常见的致病菌,初始抗感染方案推荐使用万古霉素联合美罗培南,可更大程度提高抗感染效果。
Abstract: Objective: To summarize the pathogen distribution and antimicrobial susceptibility characteristics of necrotizing fasciitis in the ethnic minority areas of central Guangxi, providing evidence for early clinical anti-infection treatment.Methods: A retrospective analysis was conducted on microbial culture and antibiotic susceptibility test results from 69 necrotizing fasciitis cases treated in the Department of Trauma Repair and Burn Plastic Surgery at Liuzhou Workers Hospital from January 2017 to December 2023.Results: A total of 77 bacterial strains were isolated from 69 cases, including 24 Gram-positive bacteria, 48 Gram-negative bacteria, and 5 fungi. Single microorganism infections predominated, accounting for 66.07%. The predominant Gram-positive bacteria were Staphylococcus aureus (10 cases, 12.99%) and Streptococcus pyogenes (5 cases, 6.49%), while the primary Gram-negative bacteria were Klebsiella pneumoniae (13 cases, 16.88%) and Escherichia coli (10 cases, 12.99%). Multidrug-resistant (MDR) strains accounted for 10.39% (18 strains) of the total isolates. Gram-positive bacteria were more sensitive to vancomycin, linezolid, and tigecycline, while Gram-negative bacteria showed greater sensitivity to meropenem, cefoperazone, and ertapenem.Conclusion: In the ethnic minority areas of central Guangxi, Klebsiella pneumoniae is the most common pathogen causing necrotizing fasciitis. For initial anti-infection treatment, vancomycin combined with meropenem is recommended to maximize therapeutic effectiveness.
