Abstract: Objective To evaluate the value of ultrasound and magnetic resonance imaging (MRI) in the diagnosis of knee osteoarthritis (KOA) by Meta-analysis. Methods A comprehensive literature search was conducted across various databases, including PubMed, the Cochrane Library, Web of Science, CBM, Embase, CNKI (China National Knowledge Infrastructure), VIP (China Science and Technology Journal Database), and WanFang Database of Medicine, to gather all relevant studies on the use of ultrasound and MRI in KOA up to April 2024. Two independent reviewers meticulously screened the literature according to predefined criteria, extracted data, and assessed the quality of the studies. The Meta-Disc 1.4 software was employed to conduct a meta-analysis of these studies, with the objective of providing a comprehensive evaluation of the effectiveness of ultrasound and MRI in the diagnosis of KOA. Results In the comprehensive analysis, a total of 14 studies involving 1613 patients were included. All included studies used arthroscopy or surgical pathology as the gold standard for diagnosis. The pooled sensitivity, specificity, and diagnostic odds ratio (DOR) for ultrasound in diagnosing KOA were 0.79, 0.82, and 20.30, respectively, with an AUC curve of 0.89 for the area under the subjects work characterization curve. In comparison, the pooled sensitivity, specificity, and DOR for MRI in diagnosing KOA were 0.88, 0.85, and 42.35, respectively, with an AUC of 0.92 under the SROC curve. Conclusion Ultrasound and MRI have good accuracy in the diagnosis of KOA, but MRI is superior to ultrasound.
