Study and evaluation of diagnostic methods for HIV co-infection with tuberculosis: a network meta-analysis

Abstract: To evaluate the diagnostic value of 6 commonly used detection methods for HIV/AIDS combined with tuberculosis infection, and to provide evidence-based evidence for clinical diagnosis of HIV combined with tuberculosis infection. Methods Diagnostic tests related to HIV and tuberculosis infection were collected from CNKI, Wanfang, Cochrane Library, PubMed and Embase databases from the establishment of each database to March 2024. After literature screening, quality evaluation and data extraction, Meta-disc 1.4 and Stata 14.0 software were used to perform mesh meta-analysis. Results A total of 21 articles were included, including 5291 patients, involving 6 diagnostic methods: TB infection T cell assay (T-SPOT.TB), Rifampicin resistance real-time fluorescence quantitative nucleic acid amplification technique (Xpert MTB/RIF), tuberculin test (PPD), TB antibody assay (TB-AB), Mycobacterium tuberculosis skin test (TST) and gamma-interferon release test (IGRA). The results of meta-analysis showed that the combined sensitivity of the 6 detection methods was 0.70 (95%CI: 0.68-0.73), 0.51 (95%CI: 0.42-0.60), 0.31 (95%CI: 0.27-0.36), 0.23 (95%CI: 0.23), respectively. 0.18-0.29), 0.43 (95%CI: 0.32-0.54), 0.57 (95%CI: 0.50-0.64), and the co-specificities were 0.79 (95%CI: 0.76-0.81) and 0.98 (95%CI: 0.98), respectively. 0.88-1.00), 0.71 (95%CI: 0.68-0.75), 0.81 (95%CI: 0.77-0.85), 0.62 (95%CI: 0.54-0.70), 0.66 (95%CI: 0.59-0.73). The results of mesh meta-analysis showed that T-SPOT.TB had the highest sensitivity and SUCRA (cumulative ranking probability curve) of negative predictive values among the 6 detection methods. Xpert MTB/RIF had the highest SUCRA specificity and positive predictive value. Conclusion Among the 6 commonly used methods, T-SPOT.TB had the highest sensitivity and negative predictive value, and Xpert MTB/RIF had the highest specificity and positive predictive value. Therefore, T-SPOT.TB and Xpert MTB/RIF may be effective detection methods for HIV and TB infection, and this conclusion needs to be further confirmed in more studies.
