Abstract: Objective To investigate the application value of musculoskeletal ultrasound combined with contrast-enhanced ultrasound (CEUS) in the diagnosis of rotator cuff injury. Methods A total of 23 patients with rotator cuff injury admitted to our hospital from January 2022 to October 2023 were enrolled in this study. All patients underwent high-frequency ultrasound examination of the shoulder joint, MRI examination of the shoulder joint, and CEUS examination of the shoulder joint one day before arthroscopy. The results of the high-frequency ultrasound, shoulder joint ultrasound contrast-enhanced imaging, and MRI in the classification of supraspinatus tear were compared, and the diagnostic efficacy of each imaging method was evaluated. Results The accuracy of the high-frequency ultrasound in diagnosing complete tear was 65.22%, the area under the ROC curve (AUC) was 0.655, the accuracy of CEUS in diagnosing complete tear was 91.3%, the AUC was 0.913, the accuracy of MRI in diagnosing complete tear was 86.96%, the AUC was 0.818; the accuracy of the high-frequency ultrasound in diagnosing partial tear was 69.57%, the AUC was 0.65, the accuracy of CEUS in diagnosing partial tear was 86.96%, the AUC was 0.871, the accuracy of MRI in diagnosing partial tear was 86.96%, the AUC was 0.842; the accuracy of the high-frequency ultrasound in diagnosing no tear was 69.57%, the AUC was 0.542, the accuracy of CEUS in diagnosing no tear was 95.65%, the AUC was 0.833, the accuracy of MRI in diagnosing no tear was 91.3%, the AUC was 0.808. Conclusion CEUS has the highest diagnostic efficacy and the largest AUC area for complete tear, partial tear, and no tear, followed by MRI, and finally, high-frequency ultrasound. There is no significant difference in the diagnostic efficacy between CEUS and MRI in the classification of rotator cuff tear, but there is a statistically significant difference among the three groups in the classification.
