The value of SWE in early liver and kidney fibrosis in MAFLD

Abstract: Objective To explore the value of shear wave elastography (SWE) in the early assessment of liver and kidney fibrosis in patients with metabolic associated fatty liver disease (MAFLD). Methods A total of 92 patients were included in this study, with 52 MAFLD patients and 40 controls. General information, laboratory data, and ultrasound data were collected and compared between the two groups. The differences between the two groups, as well as the correlations between the indicators, were compared. The value of SWE elastography in liver and kidney fibrosis in MAFLD population was assessed. Results Liver function indicators and uric acid were higher in the MAFLD group than in the control group. Aspartate aminotransferase-to-platelet ratio index (APRI) were higher in the MAFLD group than in the control group. There was no significant difference between the two groups in fibrosis 4 score (FIB-4), creatinine, blood urea nitrogen, and estimated glomerular filtration rate (eGFR). The brightness of the liver, liver-kidney ratio, and liver elasticity value were higher in the MAFLD group than in the control group. There was no significant difference in kidney elasticity values between the two groups. Liver elasticity values was correlated with alanine aminotransferase (ALT). The liver-kidney ratio was correlated with body mass index (BMI), ALT, aspartate aminotransferase (AST), alkaline phosphatase (ALP), γ-glutamyl transferase and APRI. There was no significant correlation between right renal cortex elasticity value and other indicators. Conclusion SWE helps in early identification of liver hardness in the MAFLD population. The application of SWE in early kidney fibrosis in the MAFLD population needs further evaluation.
