    Abstract: The abnormal formation of intraplaque neovascularization (IPN) in atherosclerosis (AS) is a key pathological feature of plaque instability. Superb microvascular imaging (SMI) technology provides an accurate tool for non-invasive assessment of IPN through a scoring system based on the quantity of IPN. This article systematically integrates the principles and clinical value of SMI technology, and meanwhile conducts an in-depth analysis of the molecular associations between IPN and the inflammatory factor CD147 (which mediates inflammation and matrix degradation) as well as the angiogenesis marker CD34 (which reflects angiogenic activity). It also explores the synergistic effect of SMI scores combined with biomarkers in the assessment of plaque stability, ultimately providing a theoretical basis for early risk stratification and precision intervention in AS.
