Prognostic Value of systemic immune inflammation index and rSIG shock index in elderly patients with ICH

    Abstract: Objective To explore systemic immune inflammation index(SII)combined with? Reverse Shock Index Multiplied by Clasgow Prognostic value of Coma Scale Score(rSIG))for elderly patients with intracerebral hemorrhage(ICH). Methods A total of 400 elderly ICH patients admitted to the Department of Neurosurgery of the Affiliated Hospital of North China University of Science and Technology from December 2022 to December 2024 were selected and divided into a good prognosis group(152 cases)and a poor prognosis group(248 cases) according to the 90d prognosis. Systolic blood pressure, GCS score and rSIG shock index in the poor prognosis group were significantly lower than those in the good prognosis group, while heart rate, platelet count, neutrophil, D-dimer and SII in the poor prognosis group were significantly higher than those in the good prognosis group. Logistic regression analysis was used to explore the prognostic factors of elderly ICH patients, and the predictive value of systemic immune inflammation index combined with rSIG shock index for elderly ICH patients was analyzed according to the subjects working characteristic curve. Results A total of 400 patients were included, including 152 patients in the good prognosis group and 248 patients in the poor prognosis group. Systolic blood pressure and GCS scores in the poor prognosis group were significantly lower than those in the good prognosis group, while heart rate, platelet count, neutrophil, D-dimer, systemic immune inflammation index and rSIG shock index were significantly higher than those in the good prognosis group. logistic regression analysis showed that the systemic immune inflammation index(OR=1.119,95%CI: 1.077-1.164, P=0.000)and rSIG shock index(OR=0.826,95%CI: 0.790-0.865,P=0.000)can accurately predict the prognosis of elderly ICH patients. ROC curve analysis showed that the area under curve of systemic immune inflammation index and rSIG shock index in predicting the 90d prognosis of elderly ICH patients was 0.763(95%CI: 0.715-0.811) and 0.821(95%CI: 0.776~0.867), the area under the curve of systemic immune inflammation index combined with rSIG shock index to predict the 90d prognosis of elderly ICH patients was 0.859(95%CI: 0.823-0.895), and the sensitivity and specificity were 82.66% and 72.37%, respectively. The combined AUC was significantly better than the systemic immune inflammation index and rSIG shock index alone, and the difference was statistically significant(P<0.05). Conclusion Systemic immune inflammation index combined with rSIG shock index can predict the 90d prognosis of elderly patients with ICH.
