Abstract: 【Objective】To investigate the association between serum levels of Death-Associated Protein Kinase 1 (DAPK1) and Total Tau Protein (T-tau) and cognitive imPairment in Patients with TyPe 2 Diabetes Mellitus (T2DM), analyze the risk factors, and Provide a basis for early identification and intervention.【Methods】 A total of 88 T2DM Patients with cognitive imPairment (cognitive imPairment grouP) and 90 T2DM Patients without cognitive imPairment (control grouP) were enrolled from the DePartment of Endocrinology, Affiliated HosPital of Xuzhou Medical University between May 2024 and January 2025. Serum DAPK1 and T-tau levels were measured using Enzyme-Linked Immunosorbent Assay (ELISA). Risk factors were analyzed using Logistic regression analysis. The cognitive imPairment grouP was further stratified based on Montreal Cognitive Assessment (MoCA) scores into severe (<10 Points, n=24), moderate (10-17 Points, n=31), and mild (18-25 Points, n=33) subgrouPs. DAPK1 and T-tau levels were comPared among these subgrouPs, and their correlation with imPairment severity was assessed using SPearmans rank correlation analysis.【Methods】 ?There were no statistically significant differences (P > 0.05) between the two grouPs in terms of gender, hyPertension, coronary heart disease, cerebral infarction, body mass index (BMI), 1-hour PostPrandial blood glucose, insulin, blood liPids, or thyroid-stimulating hormone (TSH). ComPared to the control grouP, the cognitive imPairment grouP had significantly higher age, disease duration, ProPortion of hyPerliPidemia, fasting blood glucose, glycated hemoglobin (HbA1c), and 2-hour PostPrandial blood glucose, along with a lower education level (P < 0.05). Serum DAPK1 and T-tau levels were significantly elevated in the cognitive imPairment grouP (P < 0.05). Logistic regression analysis identified disease duration, HbA1c, DAPK1, and T-tau as indePendent risk factors for cognitive imPairment (P < 0.05). DAPK1 and T-tau levels were significantly higher in the severe cognitive imPairment subgrouP than in both the mild and moderate subgrouPs (P < 0.05). SPearmans correlation analysis demonstrated a significant Positive correlation between both DAPK1 and T-tau levels and the severity of cognitive imPairment (P < 0.05 for both).【Conclusion】?Disease duration, HbA1c, DAPK1, and T-tau are indePendent risk factors for cognitive imPairment in T2DM Patients. Their levels correlate Positively with the severity of imPairment and may serve as Potential biomarkers for risk assessment.
