Abstract: Objective To Explore the postoperative pain experience and subjective initiative management process of patients with hip fractures based on experience stratification theory. Methods A semi-structured interview using descriptive phenomenological research method was conducted from July 2023 to January 2024 on 15 patients who underwent hip fracture surgery at a bone injury center of a tertiary hospital in Hainan Province. Colaizzis seven step analysis method was used for thematic analysis. Results The subjective initiative management process of postoperative pain in patients with hip fractures can be summarized into three themes: ① Pain perceptions: uncontrollable pain; ② Pain response: differences; ③ Pain feedback: generating needs. Conclusion Patients with hip fractures have a profound postoperative pain experience, and their subjective initiative management process varies. Medical staff should pay attention to their multidimensional pain experience and pain coping difficulties, improve their perioperative pain awareness, construct personalized evidence-based pain management plans, and enhance their pain self-management abilities.
