Observation on the Clinical Application of Integrative Medicine in the Management of Non-dialysis Patients with Stage 3-5 Chronic Kidney DiseaseXIE Wenjia,NI Jianing,ZHOU Shuyan,ZHANG Tingting,CAI Xudong

• 摘要：目的 探讨中西结合慢性肾脏病管理对CKD3-5期非透析患者临床效果。方法 收集2022年2月—2022年12月在宁波市中医院肾病科就诊的CKD3-5期非透析患者164例，随机分为对照组和管理组各82例。对照组进行常规CKD门诊管理，管理组进行CKD中西医结合管理。用配对样本t检验比较两组患者随访前后血尿酸、胆固醇、白蛋白、主观量表评分的变化，用独立样本t检验比较上述指标的组间差异，用卡方检验比较两组患者高钾血症、高磷酸盐血症、贫血、肾功能进展的发生率。随访期间每组各脱落3例。结果 管理组随访1年后的血尿酸、胆固醇水平均低于随访前，且管理组的血尿酸低于对照组（P＜0.05）；管理组高钾血症、高磷酸盐血症、贫血、肾功能进展的发生率均小于对照组（P＜0.05）；管理组KiKS评分、SF-36总评分高于对照组，中医症候总积分小于对照组（P＜0.05）。结论 中西医结合慢性肾脏病管理在CKD3-5期非透析患者中应用效果显著，能有效降低CKD进展的危险因素，控制并发症，延缓肾功能进展，提高患者的生活质量。

  Abstract: Objective To explore the clinical effect of integrative medicine in the management of non-dialysis patients with stage 3-5 chronic kidney disease.Method 164 cases of non-dialysis patients with stage 3-5 chronic kidney disease treated in the nephrology department of Ningbo Municipal Hospital of Traditional Chinese Medicine from February 2022 to December 2022 were retrospectively collected，and be randomly divided into a control group and a management group，with 82 cases in each group. The control group underwent regular CKD outpatient management，the management group underwent CKD integrative medicine management.Use paired sample t-test to compare the changes in blood uric acid, cholesterol, albumin, and subjective scale scores between two groups of patients before and after follow-up,use independent sample t-test to compare the inter group differences of the above indicators,use chi square test to compare the incidence of hyperkalemia, hyperphosphatemia, anemia, and renal function progression between two groups of patients.During the follow-up period, 3 cases were lost in each group.Result The blood uric acid and cholesterol levels in the management group after 1 year’s follow-up were lower than that before the follow-up,and the blood uric acid in the management group was lower than that in the control group（P＜0.05）;The incidence of hyperkalemia, hyperphosphatemia, anemia, and renal function progression in the management group was lower than that in the control group（P＜0.05）；The KiKS score and SF-36 total score of the management group were higher than those of the control group,and the total score of traditional Chinese medicine syndrome was smaller than that of the control group（P＜0.05）.Conclusion The application of integrative medicine in the management of non-dialysis patients with stage 3-5 has a significant effect,which could effectively reduce risk factors for CKD progression, controlle complications, delay renal function progression,and Improve the quality of life for patients.
