Abstract: ABSTRACT Objective: To analyze the risk factors for cardiovascular disease (CVD) in patients with rheumatoid arthritis (RA) by integrating multiple inflammatory markers. Methods: Clinical data of 225 RA patients hospitalized at the Second Affiliated Hospital of Zhengzhou University from January 2023 to September 2024 were collected for a retrospective analysis. Univariate and multivariate regression analyses were conducted to identify risk factors for CVD in RA patients. Results: Among the 225 RA patients, 50 (22.2%) were diagnosed with CVD. Univariate analysis showed that age, hematocrit, red blood cell distribution width (RDW), erythrocyte sedimentation rate (ESR), neutrophil count/high-density lipoprotein cholesterol ratio (NHR), and platelet count/lymphocyte count ratio (PLR) were associated with the occurrence of CVD in RA patients (P<0.05). Multivariate analysis revealed that age (OR=1.131, 95%CI=1.076–1.209), RDW (OR=1.123, 95%CI=1.006–1.272), NHR (OR=1.471, 95%CI=1.116–2.279), and PLR (OR=1.018, 95%CI=1.008–1.028) were independent risk factors for CVD in RA patients (P<0.05). Receiver operating characteristic (ROC) curve analysis indicated that the areas under the curve (AUC) for age, RDW, NHR, and PLR were 0.844, 0.797, 0.572, and 0.713, respectively. The AUC of the combined diagnostic model reached 0.898, outperforming individual markers. Conclusion: The combination of age, RDW, NHR, and PLR can be used for the early diagnosis of CVD in RA patients, improving diagnostic accuracy and patient outcomes.
