    Abstract: Population Health Management (PHM) is a population-based health management theory and tool, which mainly focuses on improving population health level and reducing health costs by strengthening individual active health management capabilities, providing prevention oriented integrated health services, realizing cooperation between multiple entities and multiple departments, and providing corresponding support measures. This study systematically combed the development of PHM theory and the core connotation, as well as the in-depth summary of the experience and effectiveness of typical foreign practice cases, with a view to provide experience and reference for China to build and improve a population-based health management system.
