Clinical application of dermatoscope combined with rapid frozen section examination in the treatment of head and face skin tumors.

• 摘要：目的 评估皮肤镜联合快速冰冻切片检查的方法在头面部皮肤肿瘤治疗中的应用价值。方法 回顾分析2019年6月至2023年12月金华市中心医院皮肤科收治的123例头面部皮肤肿瘤患者,分别采用术前皮肤镜联合术中快速冰冻切片检查方法和皮肤活检术联合术中快速冰冻两种方式进行手术治疗。统计分析比较两种方法的手术治愈率及皮肤镜图像诊断、快速冰冻切片诊断与石蜡切片的诊断符合率。结果 术前皮肤镜联合术中快速冰冻切片检查方法和术前皮肤活检联合术中快速冰冻的两种方法在头面部皮肤肿瘤治疗中效果无明显疗效差异。皮肤镜图像分析技术与石蜡切片的诊断符合率为91.36%,快速冰冻切片检查诊断符合率为98.37%。所有病例术中快速冰冻切片及术后石蜡切片均报告基底及切缘阴性,术后随访6月以上未见复发情况。结论 在头面部皮肤肿瘤的手术治疗中,皮肤镜联合术中快速冰冻切片检查的方法效果明显,可替代术前皮肤活检,值得临床推广。

  Abstract: Objective To evaluate the application value of dermatoscope combined with quick frozen section examination in the treatment of head and face skin tumors. Methods Retrospective analysis of 123 patients with head and neck skin tumors admitted to the Dermatology Department of Jinhua Municipal Central Hospital from June 2019 to December 2023. They were treated using a combination of skin biopsy and intraoperative rapid frozen section as well as preoperative dermoscopy and intraoperative rapid frozen section assessment.The surgical cure rate of the two methods and the coincidence rate of dermatoscope image diagnosis, quick frozen section diagnosis and paraffin section diagnosis were statistically analyzed and compared.Results There is no significant difference between preoperative dermoscopy combined with intraoperative quick frozen section examination and preoperative skin biopsy combined with intraoperative quick frozen section examination in the treatment of head and face skin tumors. The coincidence rate of dermoscopy image analysis technology and paraffin section diagnosis is 91.36%, and the coincidence rate of quick-frozen section examination diagnosis is 98.37%. All cases reported negative in basal margins by intraoperative quick frozen section and postoperative paraffin section, and no recurrence was found after 6 months follow-up. Conclusion In the surgical treatment of head and face skin tumors, dermoscopy combined with intraoperative rapid frozen section examination has obvious effect, which can replace preoperative skin biopsy and is worthy of clinical promotion.
