    Abstract: Compared with traditional open TLIF, MIS-TLIF utilizing the Wiltse approach and Quadrant channel technology achieves equivalent clinical efficacy in treating lumbar degenerative diseases, while significantly reducing intraoperative blood loss, postoperative pain, and hospital stay. However, the narrow surgical field in MIS-TLIF is associated with a steep learning curve, increased risks of neurological injury, and greater intraoperative radiation exposure. This review systematically examines the indications and contraindications of MIS-TLIF, evaluates its clinical outcomes, analyzes factors influencing long-term postoperative efficacy, discusses postoperative complications, and compares MIS-TLIF with other minimally invasive techniques. Finally, future perspectives for the advancement of this technique are proposed.
