The predictive value ofperipheral blood inflammatory makers for lower extremity deep venous thrombosis after hip fracture surgery in patients

• 摘要：【摘要】 目的 探讨外周血炎症指标在髋部骨折患者术后并发深静脉血栓（DVT）的检测价值。方法 选取2022年8月至2023年7月我院收治的髋部骨折并进行手术的患者，根据是否发生DVT分为DVT组和非DVT组。对两组的一般资料和血液指标进行统计分析，并通过ROC曲线评价外周血炎症指标对髋部骨折患者术后并发DVT的预测价值。结果 最终纳入样本485例，其中DVT组61例，非DVT组424例，DVT发生率为12.58%；DVT组全身免疫炎症指数血小板计数×中性粒细胞计数/淋巴细胞计数（SII）、中性粒细胞与淋巴细胞比值（NLR）和D-二聚体均高于非DVT组水平；DVT组淋巴细胞与单核细胞比值（LMR）低于非DVT组，差异有统计学意义（P<0.05）。多因素分析显示NLR、合并心血管疾病、术前卧床时间、手术方式是髋部骨折患者术后发生DVT 的独立危险因素；ROC显示NLR的诊断价值AUC=0.606，NLR＋D-二聚体联合诊断价值AUC=0.628 结论 结合髋部骨折患者NLR和D-二聚体水平，以及其术前卧床时间和合并症，对其术后DVT的发生有一定的预测价值。

  Abstract: 【Abstract】 Objectives To investigate the value of peripheral blood inflammatory makers after hip fracture surgery in patients. Methods Patients with hip fracture who underwent surgery from August 2022 to July 2023 were selected and divided into DVT group and non-DVT group according to whether DVT occurred. The general data and laboratory indicators of the two groups were statistically analyzed, and the predictive value of peripheral blood inflammatory makers on postoperative DVT in patients with hip fracture was evaluated by ROC curve. Results 485 samples were included, including 61 cases in the DVT group and 424 cases in the non-DVT group, and the incidence of DVT was 12.58%. The levels of SII, NLR and D-dimern in DVT group were higher than those in non-DVT group，and the LMR in DVT group was lower than that in non-DVT group, the difference was statistically significant (P<0.05). Multivariate analysis showed that NLR, combined cardiovascular disease, bed time before operation and surgical method were independent risk factors for postoperative DVT in patients with hip fracture. ROC showed that AUC of NLR was 0.606, and the combined AUC of NLR+ D-dimer was 0.628 Conclusions Combining the levels of NLR and D-dimer in patients with hip fractures, as well as their preoperative bed rest time and comorbidities, has certain predictive value for the occurrence of postoperative DVT
