    Abstract: Abstract Objective: To explore the health-related quality of life (HRQOL) among patients with multidrug-resistant and rifampin-resistant pulmonary tuberculosis (MDR/RR-PTB) and analyze its influencing factors. Method: Data was collected from 50 MDR/RR-PTB patients registered for treatment management in the “Tuberculosis Management Information System” in Bazhong City from 2021 to 2024 (resistance group). A 1:1 matched case-control study was conducted, selecting 50 pulmonary tuberculosis patients sensitive to anti-tuberculosis drug treatment registered in the system during the same period as the control group. A cross-sectional survey method was employed using the Quality of Life Instruments for Chronic Disease Pulmonary Tuberculosis (QLICD-PT) to measure and compare the HRQOL between the two groups. Multiple linear regression analysis was used to analyze factors affecting the HRQOL of MDR/RR-PTB patients. Results: Before treatment, the total HRQOL score, psychological function, social function, specificity module, and physiological function of the MDR/RR-PTB group were (123.06±16.67), (30.94±6.52), (26.04±4.70), (40.82±3.42), and (25.24±5.58), respectively, while those of the control group were (134.56±12.91), (35.28±4.59), (27.94±3.20), (44.16±3.10), and (27.18±4.97), respectively. Except for physiological function,there was a statistically significant difference between the MDR/RR-PTB group and the control group in terms of total quality of life score, psychological function, social function, and specific modules (t values were 3.858, 3.848, 2.337, and 5.116 respectively, P value <0.05). Univariate analysis showed that there were statistically significant differences in the total quality of life scores among patients with different genders, medical insurance statuses, monthly family income situations, presence or absence of comorbidities, lymphocyte count, albumin, C-reactive protein , and body mass index (BMI) (P<0.05). Multiple linear regression analysis revealed that the P values for the partial regression coefficient tests of patients medical insurance and monthly family income situations were both < 0.05, indicating statistical significance at the α=0.05 level of significance. Conclusion: The HRQOL of MDR/RR-PTB patients is lower than that of pulmonary tuberculosis patients sensitive to anti-tuberculosis drugs. The patients health insurance and monthly family income are potential factors affecting the HRQOL of MDR/RR-PTB patients.
