Abstract: Objective: This study aims to identify the high-level factors and paths that influence the performance of medical alliances in cities, and provide recommendations for their performance management in the country. Methods: We analyzed data from 25 medical alliances in cities using clear set of concentration comparative methods (csQCA) to identify key factors and paths that affect their performance, based on the basic elements of symbiotic theory. Results: The performance of medical alliances in cities is influenced by factors such as the collaborative system, complementary resources between organizations, business associations, economic connections, assessment incentives, government support, and information platform. We identified three types of paths for high-level medical alliances in cities: operational integration, management integration, and organizational integration. Conclusion: To improve the level of medical alliance construction in cities and promote them to become four communities, we recommend that medical alliances should focus on cooperative development of medical insurance and reform, take disciplines and health services as contacts, prioritize resource coordinated governance, improve informatization construction, and promote government assessment incentives.
