    Abstract: At present, the information management of patients with chronic diseases has encountered some problems, such as dilemma in data mutual trust and sharing, difficulty in tracing after information loss or tampering, increased risk of patient privacy disclosure, etc. Blockchain has the characteristics of decentralization, openness, autonomy, tamper proof and anonymity, which can effectively solve the data security problem in the information sharing technically. Managing the information of chronic patients through blockchain can not only improves the optimization of resource allocation and management efficiency, but also helps the government make scientific decisions. In addition, it can effectively supervise medical behavior and enhance mutual trust between doctors and patients.
