Diagnosis and Treatment of a Thyroid Foreign Body: a Case Report and Literature Review

• 摘要：甲状腺异物是临床罕见疾病，多由食管异物穿透食管壁迁移至甲状腺所致。其早期症状隐匿，易与甲状腺炎、颈部感染等混淆，漏诊率较高，可能导致感染扩散、血管损伤等严重并发症。本文报道1例69岁女性患者因鱼刺穿透食管导致甲状腺异物的诊治过程，结合影像学及手术探查明确诊断，并行甲状腺部分切除术成功治疗。通过系统回顾国内外文献，总结甲状腺异物的流行病学特征、诊断策略及治疗原则，为临床诊疗提供循证依据。

   

  Abstract: Thyroid foreign body represents a rare clinical entity, predominantly caused by migration of esophageal foreign bodies through the esophageal wall into the thyroid gland. Characterized by insidious early symptoms and frequent misdiagnosis as thyroiditis or neck infections, this condition demonstrates high rates of missed diagnosis and may lead to severe complications including infection spread and vascular injuries. This article presents a case report of a 69-year-old female patient with thyroid foreign body secondary to fish bone penetration, where comprehensive imaging evaluation and surgical exploration confirmed the diagnosis, followed by successful partial thyroidectomy. Through systematic literature review, we summarize the epidemiological patterns, diagnostic algorithms, and therapeutic principles of thyroid foreign bodies, providing evidence-based guidance for clinical management.
