Treatment of female stress urinary incontinence based on the theory of The lung governs the regulation of water pathways

• 摘要：以“肺主通调水道”为指导,探讨女性压力性尿失禁的病因病机及辨证思路。女性压力性尿失禁的病机关键在于虚实夹杂,本虚标实,肺气虚弱为本,膀胱失约为标。临床当分三期辨治:初起肺卫受邪阶段,治宜宣散风邪与固护腠理并举；若病程迁延日久,邪气深伏,形成膀胱咳证,治疗应着重补益肺卫之气以祛表邪,同时配合固摄下焦与通利水道之法；至久咳肺虚阶段,治疗重心应当转向扶正培本,或补肺气、或滋肺阴,需遵循审证求因,辨证施治的诊疗原则。

  Abstract: Guided by the theory of the lung governing regulation of water passages, this study explores the etiology, pathogenesis, and syndrome differentiation of female stress urinary incontinence (SUI). The pathogenesis is characterized by a deficiency-excess complex, with root deficiency and superficial excess, manifesting as deficiency of lung qi serving as the fundamental cause and bladder dysfunction as the superficial manifestation. Clinical treatment should adopt three-stage differentiation: In the initial stage with lung-defense system affected by pathogens, therapeutic principles should combine pathogen-dispersing methods with interstitial consolidation. During the prolonged phase with pathogens deeply latent forming bladder cough syndrome, treatment priorities should focus on tonifying lung-defense qi to expel superficial pathogens while coordinating lower jiao consolidation and water passage regulation. At the chronic stage of lung deficiency due to persistent coughing, therapeutic emphasis should shift to strengthening the bodys constitution through either lung qi tonification or lung yin nourishment. The diagnostic principle of identifying etiology through syndrome differentiation and applying treatment based on pattern differentiation should be strictly followed throughout the therapeutic process.
