血清MHR、NLR及PLR与原发性高血压的相关性研究

The correlation between serum MHR, NLR, and PLR with essential hypertension

  • 摘要:[摘要]目的 探讨在原发性高血压患者中,单核细胞绝对值与高密度脂蛋白胆固醇比值(MHR)、中性粒细胞绝对值与淋巴细胞绝对值比值(NLR)及血小板计数与淋巴细胞绝对值的比值( PLR)是否可以预测患病风险。方法 选择了2024年1月1日至2024年3月31日期间在内蒙古自治区人民医院就诊的120名原发性高血压患者作为实验组,同时选取了我院就诊的30例患者为对照组。通过比较两组的一般指标及血清学指标,利用Spearman相关性分析、单因素和多因素logistic 回归分析评估NLR、MHR、PLR与原发性高血压之间的相关性,并绘制了受试者操作特征曲线(ROC)以预测患病风险。结果 实验组在白细胞计数、中性粒细胞绝对值、红细胞计数、血红蛋白含量、血小板计数、甘油三酯、总胆固醇、高密度脂蛋白、低密度脂蛋白胆固醇、NLR、MHR等指标均值高于对照组(P<0.05)。而红细胞计数、血红蛋白含量、血小板计数、白细胞计数、中性粒细胞绝对值、甘油三酯、总胆固醇、低密度脂蛋白胆固醇、NLR和MHR与原发性高血压均呈正相关关系。通过对单因素和多因素Logistic回归分析表明,MHR及NLR均为原发性高血压发病的独立危险因素。MHR与NLR预测原发性高血压曲线下面积(AUC)分别达到0.755和0.652,两者联合预测AUC为0.813 ,灵敏度75.0%,特异度83.0%。

    Abstract: [Abstract] Objective To investigate whether the ratio of monocyte to high-density lipoprotein cholesterol , neutrophil to lymphocyte, and platelet count to lymphocyte can predict the risk of disease in patients with essential hypertension. A total of 120 essential hypertension patients admitted to the People's Hospital of Inner Mongolia Autonomous Region from January 1, 2024, were included in the study. The experimental group consisted of patients admitted to our hospital from January 1, 2020 to March 31, 2024, while the control group included 30 healthy patients admitted during the same period. A comparison of the general information and blood test results was conducted for both groups. The relationship between the variables was assessed using Spearman correlation analysis, as well as univariate and multivariate logistic regression analysis. NLR, MHR and essential hypertension .Receiver operating characteristic curves were used to predict the risk of hypertension. The experimental group exhibited significantly elevated levels of white blood cells, absolute neutrophils, red blood cells, hemoglobin, platelets, triglycerides, total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, NLR, and MHR compared to the control group (P < 0.05).Elevated levels of red blood cells, hemoglobin, platelets, white blood cells, absolute neutrophils, triglycerides, total cholesterol, low-density lipoprotein cholesterol, NLR, MHR, and PLR were all found to be associated with consistent high blood pressure. Further analysis using logistic regression revealed that MHR and NLR were identified as significant independent risk factors for hypertension. The roc curve area for MHR and NLR in predicting essential hypertension risk were 0.755 and 0.652, respectively. When combined, the roc curve area increased to 0.813, with a sensitivity of 75% and a specificity of 83%. This suggests that the combination of MHR and NLR could serve as a novel inflammatory indicator for forecasting hypertension risk.
