Prediction of Lateral Cervical Lymph Node metastasis risk in Thyroid Cancer based on preoperative Lymph Node Ultrasonographic Characteristics

• 摘要：【】目的 术前超声检查是评估甲状腺癌颈部淋巴结状况的关键手段。本研究旨在通过深入分析术前颈部淋巴结的超声特征,探索其与甲状腺癌颈侧区淋巴结转移之间的联系。方法 采用回顾性研究方法,我们搜集了2018年1月1日至2024年8月1日间在我院就诊的甲状腺癌患者数据。这些患者在术前超声检查中显示颈侧区淋巴结异常,并均经历了颈侧区淋巴结清扫术。我们对这批患者的术前淋巴结超声特征进行了详尽的分析,以明确其与颈侧区淋巴结转移的相关性。结果 本次研究涵盖了296名甲状腺癌患者,其中36人(占12.2%)的颈侧区淋巴结病理结果呈阴性,而160人(占87.8%)的结果呈阳性。结论 在本研究中,我们发现甲状腺癌颈侧区淋巴结转移的独立风险因素包括超声可疑淋巴结存在微钙化、超声可疑淋巴结存在混杂或丰富血流信号、超声可疑淋巴结存在高回声团。我们将上述风险因素进行了联合分析,其曲线下面积(AUC)为0.765(95%CI 0.683-0.846,P＜0.05)。

  Abstract: 【】Objective: Preoperative ultrasonography is a crucial tool for assessing the status of cervical lymph nodes in thyroid cancer. The aim of this study was to thoroughly analyze the preoperative ultrasonographic characteristics of cervical lymph nodes to explore their association with lateral cervical lymph node metastasis (LLNM) in thyroid cancer.Methods: We conducted a retrospective study collecting data from thyroid cancer patients treated in our hospital between January 1, 2018, and August 1, 2024. These patients exhibited abnormalities in the lateral cervical lymph nodes on preoperative ultrasound and underwent lateral cervical lymph node dissection. A comprehensive analysis of the preoperative ultrasonographic features of the lymph nodes was performed to determine their correlation with LLNM.Results: The study included 296 thyroid cancer patients, of which 36 (12.2%) had negative pathological results in the lateral cervical lymph nodes, while 160 (87.8%) had positive results.Conclusion: In this study, we
