Abstract: Objective To explore the clinical application value of fetal quantitative analysis technology (fetal HQ) in evaluating the morphology and function of fetal heart in hypertensive disorders of pregnancy. Method A total of 85 women with gestational hypertension during the gestational age of 20-40 weeks from March 2020 to March 2024 had been selected as the experimental group. According to the blood pressure, urine protein, and symptoms of the pregnant women, the experimental group was divided into 35 cases of gestational hypertension group (GH), 28 cases of mild preeclampsia group (MPE), and 22 cases of severe preeclampsia group (SPE). Additionally, 150 normal pregnant women with matched gestational age were randomly selected as the control group (NOR). The fetal HQ technique was adopted to obtain cardiac morphological and functional indicators of both groups, and statistical analysis was conducted. Result Among 260 cases of fetal dynamic four-chamber heart images, data from 235 cases were successfully collected, with a success rate of 90%. Through comparison of the left and right ventricular FAC, Tei index, and GLS of the same patient’s fetal heart, there was a statistically significant difference (P<0.05) in GLS and FAC between the left and right ventricles, with the right ventricle having a higher GLS and FAC than the left ventricle. In addition, there was a statistically significant difference in GSI in each group (P<0.05). Patients in the SPE group had rounder fetal hearts. Besides, variance analysis was performed on left and right ventricular FAC, Tei index, and GLS for each group of patients. There was a statistically significant difference in right ventricular GLS and right ventricular Tei index (P<0.05). The absolute value of right ventricular GLS in the S-PE group and M-PE group was lower than that in the NOR group, while the right ventricular Tei index in the S-PE group was higher than that in the NOR group. Conclusions Fetal HQ technique provides quick and easy quantitative assessment of fetal heart shape and function. Gestational hypertension not only changes the fetal heart morphology, but also affects the fetal heart function, and the fetal right heart system is more affected than than the fetal left heart.
