Analysis of influencing factors of depression in AIDS patients undergoing hemodialysis treatment based on Structural Equation Modeling (SEM)

• 摘要：目的：了解HIV/AIDS血液透析患者的抑郁和医疗医疗社会支持的现状，以及医疗社会支持对抑郁的作用，从而为减少人群中抑郁症的发生提供一定的参考。方法：2022年3月至2024年3月，收集我院HIV/AIDS血液透析研究对象80例。采用流调中心抑郁量表(Center for Streaming Depression Scale, CES-D)和医学医疗社会支持量表(medical social support Scale, MOS-SSS)评估患者的抑郁和医疗社会支持情况。采用结构方程模型分析医疗医疗社会支持与抑郁的关系。结果： 80名研究对象中有42名(52.5%)出现抑郁症状（CES-D评分≥16）。单因素分析结果显示，健康状况、抗病毒治疗状况、医疗社会支持对HIV抑郁有影响(P＜0.05)。简单线性回归显示，健康状况(95% CI: - 9.901~-2.635)和抗病毒治疗状况(95% CI: - 12.969~-3.394)是抑郁的影响因素(P＜0.05)。医疗社会支持总分、有形支持维度、信息与情感支持维度、社会互动合作维度、情感支持维度与抑郁存在关联(P ＜0.05)。多重线性回归分析显示，医疗社会支持与抑郁呈负相关(标准化效应值为- 0.223)。医疗社会支持越高，抑郁发生率越低。结论：收治的艾滋病血透治疗患者抑郁患病率偏高，来自医疗社会支持对缓解其病情有积极的作用。

  Abstract: Objective: To understand the current situation of depression and medical and social support in HIV/AIDS patients undergoing hemodialysis, as well as the effect of medical and social support on depression, so as to provide certain references for reducing the occurrence of depression in the population. Methods: From March 2022 to March 2024, 40 research subjects of HIV/AIDS undergoing hemodialysis in our hospital were collected. The Center for Epidemiological Studies Depression Scale (CES-D) and the Medical Social Support Scale (MOS-SSS) were used to assess the depression and medical and social support of the patients. Structural equation modeling was used to analyze the relationship between medical and social support and depression. Results: Among the 40 research subjects, 21 (52.5%) showed depressive symptoms (CES-D score ≥ 16). The results of univariate analysis showed that health status, antiviral treatment status, and medical and social support had an impact on HIV depression (P < 0.05). Simple linear regression showed that health status (95% CI: - 9.901 ~ -2.635) and antiviral treatment status (95% CI: - 12.969 ~ -3.394) were influencing factors of depression (P < 0.05). The total score of medical and social support, the tangible support dimension, the information and emotional support dimension, the social interaction and cooperation dimension, and the emotional support dimension were associated with depression (P < 0.05). Multiple linear regression analysis showed that medical and social support was negatively correlated with depression (standardized effect value was - 0.223). The higher the medical and social support, the lower the incidence of depression. Conclusion: The prevalence of depression in the admitted AIDS patients undergoing hemodialysis treatment is relatively high, and the medical and social support has a positive effect on alleviating their condition.
