  • 摘要:目的 本研究旨在建立一种高仿真的3D打印困难气道模型,应用柯式评估模式来评估该模型用于实习医生培训课程的效果。方法 根据困难气道患者的头颈胸部CT信息进行3D打印,并编写情境模拟教学教案。研究对象为30名实习医生,他们被随机分为观察组和对照组,观察组接受3D打印气道模拟培训,对照组接受传统困难气道培训。通过柯氏评估模式,从反映层、学习层、行为层和成果层4个方面对培训效果进行分析。结果 反应层:观察组学员在技能操作培训、培训内容的实效、对处理困难气道信心提升度以及达到预期目标几项中的评价明显高于对照组。学习层:观察组学员理论考核成绩平均值大于对照组,但差异无统计学差异。行为层:观察组学员的技能考核成绩明显高于观察组(P<0.05)。成果层:麻醉科教研室老师对观察组学员的评分明显高于对照组(P<0.05)。总结 3D打印困难气道模型用于困难气道管理课程的模拟训练有较好的效果,适宜推广应用。

    Abstract: Objective l The purpose of this study is to establish a highly realistic 3D printed difficult airway model and apply the Kirkpatrick model to evaluate the effectiveness of the model in intern training courses.. Methods Based on the CT information of the head, neck, and chest of difficult airway patients, a 3D printed model was created and scenario-based simulation teaching plans were developed. The study included 30 interns, who were randomly divided into an observation group and a control group. The observation group received 3D printed airway simulation training, while the control group received traditional difficult airway training. The effectiveness of the training was analyzed using the Kirkpatrick Model, focusing on the reflective level, learning level, behavioral level, and outcome level. Results Reflective level: The evaluation of the observation group in terms of skill operation training, effectiveness of training content, confidence in managing difficult airways, and achievement of expected goals was significantly higher than that of the control group. Learning level: The average score of the theoretical assessment was higher in the observation group than in the control group, but the difference was not statistically significant. Behavioral level: The skill assessment score of the observation group was significantly higher than that of the control group (P < 0.05). Outcome level: The evaluation score given by the teachers in the Department of Anesthesiology and Research to the observation group was significantly higher than that of the control group (P < 0.05). Conclusion The 3D printed difficult airway model has shown good effectiveness in difficult airway management courses, and it is suitable for widespread application.
